The androgen picture is complete rather than partial: total testosterone, free testosterone, SHBG, albumin, LH, FSH, sensitive estradiol, DHT, prolactin and IGF-1. That combination is what lets a clinician distinguish primary from secondary hypogonadism, work out whether a low free testosterone is an SHBG problem rather than a production problem, and decide whether an aromatase inhibitor is ever justified — which, on most protocols, it is not.

Around that sits a full metabolic and cardiovascular workup: lipids with ApoB, hs-CRP, fasting insulin, HbA1c, complete blood count with hematocrit, comprehensive metabolic panel, thyroid with free T3 and free T4, PSA, and iron studies.
