Most people start semaglutide or tirzepatide with a weight on a scale and nothing else. That is a mistake you cannot correct retroactively — once the drug is working, you no longer know where you started.

Why baseline labs matter
Baseline labs do three separate jobs. First, they screen for conditions that change whether or how the drug should be prescribed — undiagnosed diabetes, severe hypertriglyceridemia, impaired renal function, untreated thyroid disease. Second, they create a comparison point, so that six months later you can distinguish real metabolic improvement from weight loss alone. Third, they set the monitoring plan: you cannot detect a meaningful change in a marker you never measured.
This matters more with GLP-1 therapy than with most interventions because the weight change is large and fast. Body weight will move regardless. Whether insulin sensitivity, lipids, and liver enzymes moved with it is the question that determines whether the outcome was actually a health improvement.

Contraindication markers
Two categories sit outside the standard panel and are answered by history rather than by a blood draw. A personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia type 2 is a contraindication to both semaglutide and tirzepatide. A history of pancreatitis warrants caution and usually a triglyceride check, since severe hypertriglyceridemia is itself a pancreatitis trigger; some clinicians will also check baseline lipase, though its predictive value in asymptomatic patients is limited.
Active gallbladder disease, gastroparesis, and severe gastrointestinal disease are also worth raising before the first dose, because GLP-1 agonists slow gastric emptying by design.
Muscle preservation baseline
A substantial share of GLP-1 weight loss can come from lean mass. That is the single most consequential and least monitored risk of the drug class, because lean mass lost in your forties is not easily recovered in your sixties. A DEXA scan before starting is the best available baseline, giving you fat mass, lean mass, and visceral fat separately.
If DEXA is not accessible, the minimum viable baseline is body weight plus grip strength measured with a hand dynamometer, ideally alongside a simple performance marker such as a five-repetition maximum on a compound lift. If grip strength falls while weight falls, the protocol needs more protein and more resistance training, not a higher dose. The strength protocol covers the training side of that.
Ongoing lab monitoring on GLP-1
Baseline testing is only half the job. GLP-1 therapy changes metabolic physiology continuously for the first year, and the markers that matter shift as you move from titration into maintenance. A monitoring schedule turns the drug from something you take into something you manage.

How GLP-1 medications move lab values
Most changes are favourable and predictable. HbA1c typically falls by one to two points in people who started above range. Triglycerides usually drop substantially, LDL modestly, and HDL rises slowly. Liver enzymes tend to improve as hepatic fat clears, which is one of the more reliable secondary benefits of the class. Fasting insulin generally falls in parallel with visceral fat.
Kidney function is the value to watch rather than to celebrate. Trial data suggest renal benefit over time, but during titration — particularly at higher tirzepatide or semaglutide doses where nausea and reduced intake are common — dehydration can drop eGFR acutely. A falling eGFR alongside gastrointestinal side effects is a fluid problem before it is a kidney problem, and it should prompt a conversation rather than a dose increase.
Warning signs that warrant a dose change
Four patterns justify pausing, reducing, or reconsidering therapy: an eGFR decline of more than roughly 30 percent from baseline; ALT or AST rising rather than falling, which points away from fatty liver and toward another cause; lipase elevation accompanied by persistent upper abdominal pain radiating to the back; and a drop in grip strength or lean mass on repeat body composition testing while weight continues to fall. The last of these is the most common and the most ignored — it means the weight loss is coming from tissue you need.
Unintended rapid loss, more than roughly two percent of body weight per week sustained, is itself a signal to hold the dose rather than escalate. The goal is metabolic improvement with lean mass intact, not the steepest possible curve on the scale.
Cadence in practice
A workable default: repeat the metabolic markers roughly three months after reaching your maintenance dose, then every six to twelve months while stable. Recheck kidney function sooner if you have had a prolonged period of vomiting or reduced intake. Track body composition or grip strength quarterly rather than annually, since that is the marker most likely to deteriorate quietly.
Keep the same lab and, where possible, the same assay. Between-lab variation on fasting insulin in particular is large enough to manufacture trends that do not exist.
